Prevention & Early Intervention
Brief services for individuals exiting inpatient hospital stays, or experiencing early-stage behavioral health issues, who need support to prevent relapse and promote well-being.

Whole Person Care
Holistic wraparound services for people who are housing insecure and dealing with chronic physical/behavioral health diagnoses that are un-treated. Our Whole Person Care programs coordinate primary care, behavioral health, social services, and housing supports. This integrated approach to managing care improves physical and mental health for the individual, while utilizing public resources more efficiently.
SSI Benefits Advocacy
Care coordination service for people with mental health challenges who need assistance to obtain Supplemental Security Income (SSI) through legal advocacy.
Solano Housing Advocacy, Permanency, and Engagement (SHAPE)
Specific supported housing for people with mental health issues, helping individuals acquire housing, develop independent living skills, and integrate into their community of choice.
Job placement services to help individuals with complex needs find competitive, satisfying, and sustainable employment to uplift them from poverty.
