Case Management

 

Seniors and adults with mental illness often need someone to check in on them to make sure they are safe and that their basic needs of food, shelter, medical care, and social interaction are being met.

Case managers assist individuals with mental health issues to get the treatment they need to reduce the risk of crisis hospitalization. They ensure that co-occuring disorders, like alcholism or drug addiction are being monitored so the client is less likely to engage in behaviors that would lead to hospitalization or incarceration.

Case managers are committed to assisting frail seniors with getting nutritious meals, proper physical care, and reliable caregiver support.

Our case managers are skilled in meeting the needs of the elderly and individuals with mental illness.

Case Management for the Mentally Ill

BACS contracts with Alameda County Behavioral Health Care to provide case management for a specific number of vulnerable individuals in Alameda County.

Qualifications to services are:

  • Persistent mental illness
  • Medi-Cal coverage
  • Our available service allocation

Case Management for Older Adults

Seniors who would benefit from case management may be referred to us by family and friends, agencies, or any concerned community member. Qualifications for service are:

  • Age 60 or older
  • Need support to remain at home
  • At risk for abuse or neglect
  • Willing to accept help
  • Income under $60,000

When a client is referred to BACS, a Case Mananger will, with the client's permission, make a home visit to assess the environment and develop a care plan. The plan might include recommendations for in-home care assistance, nutritional assistance, money management, and other services.

The Case Manager will arrange for services and provide oversight as care providers are vetted and hired. Monthly follow-up occurs for a limited period of time, typically three months, while service providers implement the care plan and offer day-to-day support.

Jeff, 41, lives at Kennedy House, one of BACS’ Supported Independent Living Programs. Jeff is also in our Case Management program. “They’ve helped me a lot because I can live on my own now, and I didn’t used to be able to,” says Jeff. “My life is a lot better now than it used to be. I don’t think about it a lot, but I guess it’s quite an accomplishment.”
